Requirements

- NY Medicaid ID required
- NP: Minimum 3,600 hours of experience
- PA: Supervising physician assigned
- COVID vaccination required
- Recent physical (within 12 months)
Clinical Responsibilities
- Conduct in-home health assessments for patients
- Complete structured evaluations (non-diagnostic, non-treatment role)
- Ask standardized assessment questions and document responses
- Review patient history and current health status
- Submit completed documentation in a timely manner
Visit Structure
- ~90 minutes per visit total:
- 30 minutes: travel prep, patient outreach, chart review
- 30 minutes: in-home assessment
- 30 minutes: documentation and submission
